How to apply for HDU Card

1. Fill out the application form.

2. Please get the signature of the responsible person of the International Office (Add:Room 417, Office Building, Tel: 86878576) on this form

3. Submitthis form, your passport copy and a picture (the same size as your passportpicture) to the Logistics Company (Add: The Service Hall at the back of the No.3 cafeteria).

4. Fees:RMB20 for the cost of the card. Students deposit RMB into the card at theService Hall.

5. Timeto pick up: 1 work day

6. The HDU student card is applicable at the cafeteria and the library. It is alsoapplicable to buy cell-phone card, internet card (the flash card) and bus/train ticket at the Service Hall.
